Problem with a federal agency

If you have a problem dealing with a federal agency, my office may be of assistance to you.  To effectively and efficiently work on your request, please complete and sign a Privacy Release form (click here to download form), provide a detailed account of the problem and describe any actions already taken.  Include copies of all documentation that support your claim or may be helpful, such as letters to and from agencies as well as medical or other forms.  Mail all materials to my Mobile office at 11 N. Water Street, Suite 15290, Mobile, AL  36602 or fax them to (251) 690-2511.
